9 Connecticut Hospitals Earn ‘D’ Or ‘C’ Grades In New Safety Ranking: Full List

A newly released Fall 2025 Hospital Safety Grade report shows that several Connecticut hospitals received below-average scores for patient safety. The Leapfrog Group, a national watchdog focused on healthcare transparency, assigns letter grades to hospitals based on preventable medical errors, injuries, accidents, and infections. While 14 Connecticut hospitals earned top “A” grades, others received “B,” “C,” or even “D” scores in this latest update.

The report also highlighted “Straight A” facilities—hospitals that have consistently earned an A grade for more than two years in a row.

System-Wide Findings

For the first time, The Leapfrog Group examined how hospital performance relates to their membership in larger health systems. According to the report:

  • 90% of hospitals with Safety Grades are part of a health system.
  • 94% of A-rated hospitals are system-affiliated.
  • 95% of “Straight A” hospitals operate within health systems.
  • All 11 hospitals that have earned an A every year since 2012 are part of health systems.

The report also identified the 10 health systems with the most A and Straight A hospitals nationwide, including HCA Healthcare, CommonSpirit Health, AdventHealth, Kaiser Permanente, AdvocateHealth, Prime Healthcare Services, the Defense Health Agency, Lifepoint Health, Northwell Health, and Sutter Health.

Connecticut ranked 4th nationwide for its percentage of A-rated hospitals. Four states—Iowa, North Dakota, Vermont, and Wyoming—had no A-rated hospitals at all.
